4 Legged Friends’ Registration Forms for Cats & Dogs

Thank you for trusting 4-Legged Friends with the health and happiness of your furry friend! Once your dog or cat is registered (and your dog has passed a temperament test and is approved for day play), you can make online reservations at your convenience.

We accept new dog and cat clients that are between the ages of 12 weeks and 12 years old. We recommend in home care for pets over 12 years old that have not experienced boarding with us prior.

Canine Forms

To begin registering your dog, submit the following form and we will contact you to set up a temperament test. All assessments are done by appointment only. Call 734.522.7297 with any questions.

Feline Forms

To begin registering your cat, submit the following form (please upload current vaccination records). Please contact us to set up your boarding reservation if you have not done so. Call 734.522.7297 with any questions.
